Health Plan
JSI's health insurance plan is available to all
regular full-time employees (working 30 hours or more each week)
after 90 days "Introductory Period" with 100% cost coverage for
employee. Employees will be allowed to enroll dependents (spouse,
children or domestic partner) however, you will be responsible
for insurance premium of his/her dependents. Employee may select
HealthNet HMO or PPO, Kaiser, or Harvard Pilgrim Health Care
(Boston location only) for health and vision benefits, and Guardian for dental
coverage.
- HealthNet HMO-100% coverage for preventive
care, inpatient services, laboratory, and X-rays. $15. copayment
for office visits, chiropractic, well childcare with no annual
deductible. $10. generic formulary and $25. brand Name Formulary,
and $35. for non-formulary prescription drugs.
- HealthNet PPO- allows employees to choose
between PPO and non-PPO providers to receive care.
- PPO - the majority of the services are paid
at 100% after $15. copayment with maximum $250. individual/$750. family deductible.
- Non-PPO providers are generally paid at 70%
of professional, hospital, mental health services, chiropractic,
and acupuncture benefits, after the annual deductible has been
satisfied ($250. individual/$750. family).
- Kaiser Office and hospital visits at Kaiser
facilities only. Most services are covered at either 100% coverage
or 100% after a $20. copayment.
- Harvard Pilgrim Health (Boston only)- Employees
choose to receive care from approved providers. Most services
are covered at 100% after a $15. copayment. $10. generic formulary,
$20. for brand Name Formulary and $35. for non-formulary .
Vision
HealthNet HMO and PPO-100% coverage for exams
every 12 month. Frames covered up to a maximum of $85. every 24
months. Contact lenses covered up to a maxiumum of $250 and $120.
for cosmetic .
HealthNet Non-PPO - $45. allowance fro exams
every 12 months and frames fro every 24 months. Contact lenses
have an allowance of $250. for necessary and $105. for cosmetic.
Kaiser - with $20. copayment for exams every
12 months. Frames and lenses have an allowance of $175. every
2 years.
Dental
Employee may select either DMO or PPO
- Guardian DMO-with neither annual deductible
nor lifetime maximum. 100% after a $10. copayment for most other
services.
- Guardian PPO- Permits employee to select between
PPO or non-PPO providers with an annual deductible of $50. individuals/$150.
family and $2000. annual maximum. There is a $2000. lifetime
maximum for orthodontia per individual, or dependent children
under age.
- Guardian PPO - there is a 100% coverage for
preventive care, 90% of basic services ), 60% of major services.
- Guardian Non-PPO providers-100% for preventive
care, 80% coverage for basic services, and 50% for major services.
Life Insurance
JSI provides $20,000 of basic life insurance for
every employee. Optional benefit increments available to employees
who are interested in purchasing additional amounts of life insurance
up to $250,000.
Life Protection Insurance
We offer extra basic life insurance which consists
of accidental death, cancer, and disability insurance. Employee
may purchase one or any combinations of three available options.
Additional coverage can be purchased for individual or family
coverage.
